If you have been referred to us by your dentist or have been recommended to seek periodontal care, please contact our office to schedule a consultation. It is important to note that you do not require a referral from your dentist as we are always happy to welcome new patients. If you are concerned about the condition of your gums, you have a strong family history of periodontal disease or you would like more information on dental implants, please fill out the referral form below and bring it to your first consultation appointment.
